Job Overview

The RN Care Manager is a member of the Patient Aligned Care Team (PACT) that includes a physician, registered nurse, LVN and medical support assistant. Together this team works to provide comprehensive, health maintenance services to Veterans by executing the patient-centered care plan. The RN Care/Staff Nurse is responsible for collaborating with services at VA and externally to facilitate care transition and provide excellent patient care outcomes.

The practice area for the RN Care Manager is within the primary care/infectious disease and women's health care clinics.

The RN Care manager coordinates the care provided by the outpatient clinics. The duties of this position include, but are not limited to:

  • Assist the provider in managing a panel of patients and proactively reaching out to "high risk" patients
  • Perform both face-to-face and telephone triage for Veterans
  • Work with team to manage patient workload in a timely manner
  • Assist with care coordination for patients transitioning from or to community facilities, specialty services and home care services.
  • Coordinating tests, arranging accommodations for Veterans travelling to appointments, assisting with travel arrangements
  • Assist physicians with procedures, patient schedules, medication administration
  • Provide health education and recommendations based on the patient plan of care
  • Provide ongoing wound care for patients as needed
  • Collaborate with team members to manage patient needs
  • Participation in unit-level improvement activities

The RN Care Manager reports directly to the Nurse Manager, Medical Practice.

Work Schedule:Monday thru Friday, 7am-3:30pm 8am-4:30pm or 10:30am-7pm. Saturday hours from 8am-Noon may be required to cover Veteran care needs

  • U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
  • The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. *Grade Determinations: *The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:

  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor’s of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor’s degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; ORa Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor’s degree in a related field and approximately 2-3 year’s of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year’s of nursing practice/experience; ORa Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year’s of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year’s of nursing practice/experience.

Preferred Experience:

  • BSN preferred
  • 2-3 years of recent Primary Care, Urgent care or Acute care nursing experience
  • Knowledge of community resources
  • Knowledge of the medical home model for primary care Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). *Physical Requirements: This position requires visual acuity, keen hearing, clear, distinctive speech and manual dexterity. There may be long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The RN Care Manager may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or for procedures. The RN Care Manager may occasionally be exposed to patients who are combative secondary to delirium, dementia and/or psychiatric disorders. This position potentially requires flexibility in schedule and assignments to meet Veteran care needs. *
