Role Purpose

Responsible for bringing together and coordinating the administrative/financial and clinical functions included in the overall Care Management process for all BJC HealthCare organizations. This requires development and implementation of a care coordination strategy that reflects the current healthcare environment and health policy trends while anticipating future requirements. Works to synthesize quality measures and outcomes with appropriate payment structures.


  • Manages individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.

  • Provides both clinical quality and clinical direction relating to the coordination of care at the BJC system level.

  • Leads, provides direction, and associated change management support for BJC initiatives focused on alternative care delivery models. Serves as a resource to teams in terms of clinical direction by evolving coordination of care models coupled with appropriate reimbursement.

  • Provides direction that enables a balanced alignment of desired clinical and financial goals, as well as implementation of supporting processes across case management, utilization management, and discharge planning.

  • Serves as key coordination point for the Center for Clinical Excellence and Finance/Managed Care organizations, and other BJC initiatives and departments in relation to coordination of care.

  • Defines outcome metrics associated with new models of care coordination and care delivery and designs pilots and assists with implementation of models resulting from successful pilots.

Minimum Requirements


  • Bachelor's Degree

    • Nursing

    • HC Admin/PH/Soc Work/related


  • 10+ years
