Lifetime Care Physician Medical Director - Hospice in Rochester, New York
Description The Medical Director provides over-site of physician services for the Hospice Program, in accordance with accepted standards of practice and which support the philosophy, mission and purpose of Hospice. This position manages assigned staff.
Provides clinical over-site of hospice program.
Provides supervision to other members of the medical team including Assistant Medical Directors and assigned Nurse Practitioners.
Participates in Interdisciplinary Team Meetings and admission and re-certification decisions.
Reviews patient eligibility for hospice services.
Completes certification of patient terminal illness, as appropriate.
Responsible for medical needs of hospice patient not met by the attending physician. Act as attending physician for patients referred without an MD.
Consults with attending physicians regarding pain and symptom control and with staff on difficult symptom management issues.
Participates in Agency Performance Improvement Program.
Provides home visits for Palliative Care Consultation, Patient Assessment and Intervention, patients with difficult symptom management problems, and for patients coming up for re-certification with questionable prognosis.
Provides home visits for hospice patients within two weeks of admission and occasional visits as needed to patients in hospice scatterbed(s).
Teaches medical students, residents and fellows, as necessary.
Provides regular updates to attending physicians via fax, or e-mail and interact with Long Term Care facility medical directors and physicians.
Advocates for hospice coverage with private insurers as appropriate.
Provides input to Agency leadership on Strategic Planning and Palliative Care Development.
Provides over-site for Hospice Pharmacy coverage.
Participates in community education efforts to promote the hospice philosophy of care and to improve general community and professional understanding of Hospice services.
Participates in education of skilled nursing facilities, physicians, and other community health care providers, regarding hospice and palliative care.
Presents In-services to the medical community as requested and as available.

Regular and reliable attendance is expected and required.
Performs other functions as assigned by management.
Doctor of Medicine or Doctor of Osteopathy. Requires board certified physician, preferably current American Board of Hospice and Palliative Medicine (ABHPM) certification. If not currently ABHPM certified, must be willing to sit for ABHPM certification within 1st year of employment, or when eligibility becomes effective.
Background in one or more of the following: Oncology, Geriatrics, Internal or Family Medicine. Expertise and confidence in Palliative Pain and Symptom Management.
Readily accessible to staff, patients and attending physicians.
Proactive in building professional relations.
Self-directed and self-motivated.
Possess ability to travel.
Excellent interpersonal and verbal communication skills.
Ability to work effectively within Lifetime Care Hospice Interdisciplinary Team.
