Child & Family Services of RI Case Manager - Elder Services ( BiLingual- Spanish) in Providence, Rhode Island
Promote independent living by providing counseling, advocacy, education and support systems to the elderly living in the community. Maintain a working knowledge of community resources and a working knowledge of payer requirements. Assist in telephone screening of elders and in-home assessment of Senior's needs. Develop plans of care to address needs and problems identified, and to incorporate services needed to enhance current support system. Evaluate the client’s response to the plan of care and update the plan based on client’s response. Provide support to client regarding daily living issues; conduct home visits as necessary. Respond to Protective Service Cases.
Bachelor Degree in Gerontology, Social Work, or related field with Elder experience. Valid driver’s license required. Bilingual in Spanish and English required. Knowledge of the structure and content of the English & Spanish language including the meaning and spelling of words, rules of composition, and grammar.
