Responsibilities: • Establish and maintain communication and working relationship with coordinated Entry System Housing Navigation staff to facilitate referrals and housing placement • Provides assistance in linkages for homeless individuals in the program and maintains case load • Completes weekly case conferencing with Program Manager, and maintains complete and accurate documentation of services in accordance with federal, state, and county • Meets with clients on a regular basis to establish goals, and action steps towards those goals, and to monitor and assess progress towards achieving permanent housing goals • Develops and maintains inter-agency and community relationships • Maintains strict confidentiality of employee, client, company, and vendor data, as well as handling sensitive and critical information with professionalism and discretion • Other duties as assigned

Requirements: • Bachelor's degree preferred • 1 - 3 years experience working with homeless individuals • Demonstrate excellent computer skills with proficiency in Microsoft office • Excellent oral, written, organizational, and motivational skills

Additional Requirements: • Bilingual (Spanish) preferred • Valid California driver's license, insurance and reliable transportation • Successful completion of tuberculosis and background screening
