Job Summary: The Case Management Lead position provides supervision and coordination of the Case Manager Program within the Case Management Department. The Lead is responsible to assist with hiring, completing performance evaluations and competency assessments of all Case Management staff per HCA policy and following the disciplinary process with the assistance of the Director. This position, along with staff designee, provides orientation to new employees. Lead assures that staff stays current with practice guide lines through use of continuing education, training and provision of clinical supervision. It is expected that this position collaborates within the Case Management Department, nurse managers, staff nurses, physicians, ancillary departments and community resources to ensure that a safe, acceptable discharge plan is in place for all patients seen per policy.

Position Responsibilities: 1. Administers various human resource processes and procedures for the Case Managers to include PMP’s, coaching, disciplinary action according to policy with assistance from the department director.

  1. Assists Director to maintain compliance with HCA, federal, state and other governing bodies such as Joint Commission. This may involve assisting with projects, review and/or developing policies, and conducting periodic review of Case Manager activities to ensure compliance with policies and procedures.

  2. Assists with the hiring and onboarding of new employees

  3. Participates in developing department goals/objectives employee engagement action planning and implementing Studer initiatives.

  4. Recommends new approaches or processes to effect continual improvements in efficiency of department and services performed to promote quality patient care.

  5. Demonstrates critical thinking abilities with proactive decision-making and problem-solving skills.

  6. Provides daily leadership, support and guidance to Case Managers, including coordination of staffing needs and helping establish equitable caseloads within the department.

  7. As indicated, provides assistance to Case Managers as they work with patient and Interdisciplinary Team to arrange for post-acute needs and dealing with barriers as they arise.

  8. Provides feedback to director regarding suggestions for process improvement, barriers to discharge, payer issues, and medical necessity issues to promote quality outcomes.

    1. Works with Director to analyze data related to Case Management metrics and develop/implement processes to make improvements as indicated (LOS, Avoidable Days, Opportunity Days, MOF compliance).

    2. Insures Case Managers have the tools they need to perform their daily work.

    3. Promotes collaboration within the department to minimize account denials.

    4. Participates in preparation for Utilization Review meeting.

    5. Acts as a liaison between Case Management and other departments

    6. Acts as administrative liaison for Case Management and provides leadership to the department in Directors absence

    7. Participates on hospital committees as assigned

    Qualifications: Current Tennessee RN licensure Current BLS certification 3-5 experience in acute care case management required
