The Senior Manager of Clinical Case Management is primarily responsible for the daily management and support of the Corporate Case Management strategies for discharge planning. The Senior Manager will act as a subject matter expert and will facilitate process standardization to promote improved outcomes, maximize resource utilization, and improve organizational efficiencies. As an essential element to the overall success of the Case Management Program, the Senior Manager of Case Management, Discharge Planning consistently demonstrates a strong work ethic and commitment to excellence. This position requires the ability to function with minimal supervision in the rapidly changing environment of Case Management. The Senior Manager will work under the supervision of the Director of Clinical Case Management.

· Provides daily support to Facility and Division leadership, and identifies areas of opportunity for continuous improvement including ongoing education and resources as needed

· Supports the optimization of case management technology for discharge planning

· In conjunction with the Director of Clinical Case Management, is responsible for collecting, analyzing, and evaluating discharge planning outcomes and operations data. Uses the data from reports to facilitate the identification of gaps and support improvement opportunities

· Prepares educational resources for staff development and to make appropriate changes as needed to keep department current with new regulations, policies and trends in the field of Case Management discharge planning

· Participates on assigned projects, committees or initiatives, promoting effective communication between the entities and the department leadership.

· Assists the Director of Clinical Case Management in the development and maintenance of evidence based tools

· Provides ongoing daily support and subject matter expertise in focus studies deployed

· Provides input into the role of the social worker as it relates to functions of discharge planning, patient and family advocacy, psychosocial support and advance directives

· Offers creative suggestions to program compliance obstacles

· Provides ongoing evaluation and assessment of Facility practices and industry changes to ensure the enterprise remains current in processes related to Discharge Planning requirements

· Collaborates with other members of the Corporate Case Management team to develop policies and procedures to support systems, standards of practice, regulatory standards, and evidence-based best practices in support of discharge planning

· Assists in the delivery of instruction as needed and as requested by the Corporate Case Management team

· Conducts ongoing research to remain current in evidence-based best practices

· Works in collaboration with Corporate Compliance


· RN with current state licensure, BSN (Masters preferred) or Masters in Social Work


· Strong case management and discharge planning background with 6 years acute care experience

· 2-3 years in Case Management

· In-depth knowledge of case management, clinical resource management, social work, discharge planning in the acute care setting along with current evidence and best practice impacting discharge planning

· Ability to analyze data and employ approved management techniques and statistical tools to obtain maximum effectiveness, efficiency and information

· Ability to interpret established policies into operating procedures and execute discharge planning programs

· Knowledge and understanding of governmental and The Joint Commission regulations that support and effect development of departmental policies, procedures and standards for discharge planning

· Knowledge of information systems to support the evaluation of current technology as well as future needs

· Ability to effectively coordinate approved discharge planning functions and activities to maximum cooperation and efficiency

· Ability to direct, instruct and advise in the approved methods, procedures and practices employed in effective discharge planning

· Ability to receive and react effectively to day-to-day problems presented in discharge planning

· Demonstrates effective organizational, oral and written communications, problem solving, program development, computer, strong leadership and team building skills

· Must be proficient to expert in use of software conducive to developing and presenting program deliverables including, but not limited to, the Microsoft Office Suite

· Ability to establish and maintain collaborative and effective working relationships with a focus on the importance of teamwork

· Ability to establish priorities and meet deadlines

· Demonstrates leadership skills which model analytical and critical thinking abilities, problem resolution, proactive decision-making, negotiation skills, consensus building, and effective resource utilization

· Experience in performance improvement, preferred

· Up to 40% travel required
