HCA, Hospital Corporation of America ACO Care Coordinator in Foster City, California


The ACO Care Coordinator’s primary function is to support the ACO Care Managers and the Medical Management team to manage patients across the care continuum. The ACO Care Coordinator will assist patients to ensure they receive high quality, coordinated care and, as necessary, develop their own patient-centered action plan to support improved health. The ACO Care Coordinator is part of a multi-disciplinary team that helps patients develop problem solving and self-management skills.

The ACO Care Coordinator will implement interventions that will prevent unnecessary admissions, readmissions, and overutilization of the healthcare delivery system. And, work with the Medical Management team to meet Triple Aim: Better care, better health, and lower costs.

DUTIES AND ESSENTIAL JOB FUNCTIONS 1. Initiate and coordinate a multi-disciplinary team approach to care management which includes patients, payors, and providers across the care continuum.

  1. Work closely with the ACO Care Managers to target the high-risk, chronically ill patients and facilitate their enrollment in appropriate care management programs.

  2. Help patients to identify barriers to self-care and additional services that may be needed, such as home health care, caregiver support or community services.

  3. Respond to physician referrals for care coordination.

  4. W ork with physicians and their office staff to help coordinate care for patients.

  5. Document assessments and encounters.

  6. Answer general care coordination questions.

  7. Promote patient self-management skills. SKILLS

  8. A comprehensive understanding of the health care delivery system.

  9. Ability to listen and discover what motivates a client to adopt habits that are conducive to a high quality of life.

  10. Proficient computer skills particularly documentation, work processing, data entry, spreadsheets and electronic communication.

  11. Organized and flexible.


  12. Certification as a Medical Assistant and/or equivalent experience in a clinical setting.

  13. Minimum 3-5 years of previous experience in a managed care organization or other comparable experience.

  14. Experience and competence with Microsoft Office.

  15. Excellent telephonic skills.

  16. Ability to prioritize tasks and thrive in a highly productive environment.

  17. Ability to communicate effectively, both verbally and in writing.

  18. Familiarity with chronic disease management.

  19. Familiarity with current clinical practices for adult and pediatric clients.
