Job Functions

· Provides education on best practices and changes to practice standards to all members of the medical team who provide medication services to ensure positive patient outcomes. · Interpret orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system · Accurately compound and dispense pharmaceuticals · Provide clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed. · Develop, manage and mentor pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as determined appropriate based on patient population and literature guidelines · Document all clinical activities and interventions accurately and completely. Responsible for mentoring and education as well as ongoing analysis of interventions to insure accurate and appropriate documentation and provides summary reports · Support and participate in the goals of the organization’s quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives · Proactively maintains clinical knowledge using all available resources to ensure accurate knowledge of current evidence based standards of care. · Acts as Director’s representative/expert on medication events that occur throughout the hospital. · Responsible for design, analysis and reporting of adverse drug reactions, medication errors and pharmacy quality initiatives. Mentors others in their roles for these programs · Develop and document staff competency on an ongoing basis and develops improvement plans when deficiencies in competencies are identified · Maintain professional competency sufficient to meet current and future needs of the facility · Develop and manage specialty pharmacy practice programs as related to clinical pharmacy services · Provide basic technical support for automated dispensing machines and other pharmacy computer systems · Train and participate with Disaster Team drills and real-event response as assigned · Collects and analyses data from various reports with the goal of identifying opportunities for process improvements. · Maintain appropriate controlled substance records. Work with the Pharmacy Director to identify and develop processes to meet or exceed all controlled substance diversion prevention and monitoring standards. · Comply with all regulatory standards. Participate in quality and regulatory reviews and audits · Supervise and assist pharmacy technicians and other support staff effectively · Responsible in conjunction with pharmacy director and operations manager for staff performance evaluations · Responsible for routine publication of the pharmacy newsletter and/or other forms of written and electronic communication · Provides oversight of staff scheduling · Participate in the design and update of pharmacy computer system programs relating to pharmacy, nursing and physician programs · Uses available data to educate providers, nurses and pharmacy staff on best practices to standardize care. · Performs DUR and MUE on a variety of medications to evaluate prescribing practices to ensure evidence based practices are being followed. · Coordinates all educational activities for the pharmacy department while ensuring continuity of clinical activities on all shifts. · Responsible for setting agenda, maintaining minutes and creating meeting packets for the P&T committee. · Acts as a point person for physicians seeking guidance regarding pharmacy policy and procedures. · Rounds on clinical units daily to increase pharmacy visibility and provides resources to nursing and physicians as needed. · Acts as a staff pharmacist if needed to ensure continuity of patient care


Education: Graduate of a college of pharmacy with BS or Pharm. D accredited by the American Council on Pharmacy Education. Licensure: Licensed Pharmacist in the Commonwealth of Virginia Experience: 1 year Residency preferred or Minimum of 3 years hospital experience as a Pharmacist
