This is a Divisional Management position responsible for analyzing and directing all Health Homes Operations with supervisory responsibilities for all Salvation Army Corps and hubs implementing Health Homes Care Management program across the Empire State Division. This position is accountable for program development of the Health Homes Care Management program including: implementation, administration and program design. This position is part of the leadership team responsible for the development and implementation of strategic planning initiatives of this program for the Empire State Division.


  • Administrative/management and supervision of program directors/supervision functioning as team leader and role model in problem solving efforts, provision of clinical/program services and team building development.

  • Maintenance for all pertinent monthly statistics, record keeping requirements, funding source reports, funding/grant development and CQI (continuous quality improvement)

  • Oversee the development and design of the program in accordance with the needs of the each community across the state; maintain the image of the program within the state with active participation with state wide community groups.

  • Develop and follow an implementation plan for a statewide launch of Heath Homes Care Management.

  • Create and implement program polices through current Divisional approval processes, providing direction to staff and linkage with all Salvation Army programs, overseeing staff development on all levels, direct services, students and volunteers.

  • Assure for revenue generation and monitor any barriers to the proposed budget.

  • Oversee budget expenditures and purchases for the entire statewide program.

  • Assure that all care management activity across the Division is consistent with the principles, regulations, and policies associated with Health Home care management.

  • Assure care managers across the Division complete and maintain necessary training.

  • Evaluate impact of care managers’ activity on the Medicaid redesign goals, using QI data and observation; assist care managers as appropriate in improvement of performance.

  • Maintain contracting and all correspondence with CHUNY and other Health Homes in New York


  • MSW/MA and minimum six years experience in clinical field required. Experience working with youth and families and knowledge of family systems preferred.

  • Clinical license and two or more years of supervisory experience preferred.

  • Experience with Medicaid funded programming preferred.

  • Work is substantially complex, varied and requires the interpretation of technical and detailed guidelines, policies and procedures.

  • Decisions have a significant effect on achieving the goals and objectives of the organization.

  • Applicant must be fluent with the concepts of family-driven/youth-guided care, the concept of family as natural care manager, and understands that improvement in the health care status of the youth is the single most important metric.

  • Applicants must also be able to maintain a detailed awareness about the development and execution of multiple Plans of Care.

  • They must be able to communicate effectively with a diverse caseload of supervisees and comfortable with the analysis and utilization of Quality Improvement data.

  • Applicant must understand care manager job descriptions and variation according to acuity and service specialty (e.g., EI) as appropriate.

  • They will have direct responsibility for revenue generation which will be evaluated quarterly.

  • Management, program development and supervision experience required.

  • Excellent written and verbal communication skills, data analysis, knowledge and experience with EMR, and awareness of community resources.

  • Must have valid driver’s license that meets The Salvation Army insurance requirements.

  • Must have access to own vehicle during work hours.
